It was first published in 1987 and is now firmly established as a companion to all medical students, young doctors and healthcare professionals. Looking through an almost complete set of the previous editions of Gray’s Anatomy, I am struck by the marked difference in size between the first and fortieth editions. That progressive increase in girth has occurred pari passu with ground-breaking advances in basic science and clinical medicine over the past 155 years. Anatomy has become a far wider discipline than Henry Gray, Henry van Dyke Carter or any of their students could have envisaged. Fields such as cell biology, molecular genetics, neuroanatomy, embryology and bioinformatics either had not emerged or were in their infancy in 1858. Techniques that today inform our view of the internal landscape of the body – such as specialized types of light and electron microscopy; imaging modalities, including X-rays, magnetic resonance imaging, computed tomography and ultrasonography; the use of ‘soft’ perfusion techniques and frozen-thawed, unembalmed cadavers for dissection-based studies; and the advances in information technology that enable endoscopic and robotic surgery and facilitate minimally invasive access to structures previously considered inaccessible – were all unknown. As each development entered mainstream scientific or clinical use, the new perspectives on the body it afforded, whether at submicroscopic or macroscopic level, filtered into the pages of Gray’s Anatomy: for example, the introduction of X-ray plates (twenty-seventh edition, 1938) and electron micrographs (thirty second edition, 1958).

In the Preface to the first edition, Henry Gray wrote that ‘This Work is intended to furnish the Student and Practitioner with an accurate view of the Anatomy of the Human Body, and more especially the application of this science to Practical Surgery.’ We remain true to his intention. An appropriate knowledge of clinically relevant, evidence-based anatomy is an essential element in the armamentarium of a practising clinician;indeed, ‘If anything, the relevance of anatomy in surgery is more important now than at any other time in the past’ (Tubbs, in Preface Commentary, which accompanies this volume).

In my Preface to the fortieth edition, I intimated that the book was quite literally in danger of breaking its binding if any more pages were added. In order to avoid this unfortunate occurrence, the forty-first edition contains a significant amount of material that is exclusively

electronic, in the form of 77,000 words of additional text, 300 artworks and tables, 28 videos and 24 specially invited commentaries on topics as diverse as electron microscopy and fluorescence microscopy; the neurovascular bundles of the prostate; stem cells in regenerative medicine; the anatomy of facial ageing; and technical aspects and applications of diagnostic radiology. In keeping with the expectation that anatomy should be evidence-based, the forty-first edition contains many more references in the e-book than could be included in the

thirty-ninth and fortieth printed editions.

It is difficult to believe that this preface signifies the fourth edition of Ganong’s Review of Medical Physiology that our author group has overseen, and the 26th edition overall of this important reference work aimed at medical and otherhealth professional students. As always, we have tried to maintain the highest standards of excellence that were promulgated by the original author, Fran Ganong, over the 46 years where he served, remarkably, as the sole author of the textbook.

In this new edition, we have cast a fresh eye on the pedagogical approach taken in each chapter and section, and have focused particularly on including only material that is of the highest yield. We have thoroughly revised the learning objectives for every chapter, reorganized and updated the text to ensure that all objectives are clearly addressed in a logical order, aligned chapter summaries so that the take-home messages quickly address each learning objective in turn, and expanded the number of review questions so that readers also have the ability to check their understanding and retention of every objective covered. As a discipline evolves and new information emerges, there is a tendency simply to concatenate these concepts such that chapter structure degrades inevitably over time. With in-depth discussions amongst the author team and significant “spring-cleaning,” we believe we have freshened and simplified the volume while also making sure that important new developments are incorporated. We are particularly excited to have a physician-scientist on the team, who can guide us overall to focus on material that is of most benefit to those preparing for a career incorporating patient care. We are most grateful for the past contributions of Scott Boitano, PhD, whose other obligations meant that he could no longer serve as an author. The 12th edition follows in the footsteps of the 11th edition by reflecting the dynamic changes occurring

in medicinal chemistry. With increased knowledge of the disease process and the identification of the key steps in the biochemical process, the chapters have been updated, expanded, and reorganized. At the same time, to streamline the presentation of the content, some topics were combined into existing chapters. Section 1 addresses the general principles of good interaction with patients, from the basics of taking a history and examining, to the use of pattern recognition to identify spot diagnoses.

Section 2 deals with symptoms and signs in specific systems and Section 3 illustrates the application of these skills to specific clinical situations. Section 4 covers preparation for assessments of clinical skills and for the use of these skills in everyday practice. An expertly performed history and examination of a patient allows the doctor to detect disease and predict prognosis, and is crucial to the principle of making the patient and their concerns central to the care process, and also to the avoidance of harm from unnecessary or unjustified tests.

From my experience as a teacher and an examiner in medicine, I feel that many candidates, even brilliant ones, fail to succeed in long cases due to lack of adequate technique of taking history from the patient, analysis of symptoms, inapt presentation and inability to answer questions related to the case. In spite of good theoretical knowledge, many of them are unable to effectively communicate with the patient. This is because they do not prepare themselves in an examinationoriented manner. It is important to acquire knowledge from textbooks, but it is equally vital to know what to expect in examinations, how to face them and to practice with that in mind. Goalbased preparation, systematic practice and repeated exposure to different cases and rehearsal of

presentation in front of others is the secret to success. With this in mind, I have written this book to help students prepare themselves in an examination-oriented manner.

It includes a complete outline of a comprehensive medical encounter including history taking and physical examination. The best way to get the most out of this book would be to practice a real case in the ward and then go through the relevant example given in the book. Students should notice how the case has been presented, the important clinical points that are mentioned and then familiarize themselves with the relevant questions and answers. They can make small groups where one will present a case in front of others and then answer questions asked by them. This will help them to be confident and fluent during the real examination.



In examination, a patient may have a single disease, or may be suffering from multiple diseases. For the purpose of preparation and simplicity, I have prepared this book using a prototype single

case of a disease condition.



I do not claim that this book is enough for the whole clinical medicine and one should consult

standard textbooks for learning. But for examination purpose, this book will be a valuable tool for

optimum preparation.

A candidate needs to prove that he is not only knowledgeable but also competent. I hope that

this book will help to promote such an approach.
